Family Action - NE Hants & Farnham Social Prescribing Project
Family Action transforms lives by providing practical, emotional and financial support to those who are experiencing poverty, disadvantage and social isolation across the country. We have been building stronger families since 1869 – today working with over 60,000 families a year in our 200 community-based services. Reaching out to those in need, we strengthen families and communities, build skills and resilience and improve the life chances of everyone we work with.
ABOUT US: 

Family Action have been commissioned by NHS North East Hampshire & Farnham to provide a social prescribing service.
Social prescribing is about giving someone access to activities, social groups, services and advice based on their needs and interests. The aim is to improve their sense of wellbeing, which in turn can often have a positive impact on their health.

WHY OUR WORK MATTERS: 

We are supporting clients in their post-lockdown journeys in mental and physical wellbeing. The Covid pandemic has left many people feeling isolated, lonely and anxious about going out again. Our innovative and successful service makes a real difference to people’s lives through giving someone encouragement and opportunity to access activities & social groups & the practical and/or emotional support to make and sustain positive changes in their life.
